Scutt Trucking LLC will hire you as a self-employee. Scutt Trucking LLC will only provide you with a 1099 form at the end of the year. You will be responsible for paying your own taxes, as well as benefit coverages. If you agree, check the box below:

Authorization for driving record check

I hereby authorize Scutt Trucking LLC to check my driving record and qualifications for purposes of evaluating whether I am qualified for the position for which I am applying.

Driver Fees & compensation Acknowledgement

Please review the following company fees, requirements, and safety incentives before submitting your application.

ELD DEVICE

Drivers are required to have an ELD device. The cost to order the required ELD device is $230.55, which is the responsibility of the driver.

ESCROW

A total escrow balance of $2,000.00 is required. $500.00 will be deducted from each paycheck until the required $2,000.00 escrow balance has been reached.

DRUG TEST

The required drug test costs $275.00 and is the responsibility of the driver.

DOT SAFETY CHECK INCENTIVE

Scutt Trucking LLC recognizes and rewards drivers for maintaining safe and compliant equipment.

• Drivers who pass a DOT safety check will receive a $300.00 safety reward.

• If a driver fails a DOT safety check, $300.00 will be deducted from the driver's paycheck, subject to applicable company policy and law.

By submitting this application, I acknowledge that I have reviewed and understand the ELD, escrow, drug testing, and DOT safety check fees and requirements described above. I understand that applicable deductions and/or payments will be handled in accordance with company policy and applicable law.
